Pastoral Care Discussion Forum

Guidelines for recording information following pastoral visits
Can anyone share information regarding the above topic please?
We are reviewing how we record information following pastoral care visits. Currently we record details of the visit in a password protected area of My ChurchSuite that can be viewed by clergy and members of the pastoral team.
Following recent discussion the issue of consent was raised - do others ask consent from those people being visited to record the details of the visit? We recognise that if a safeguarding issue emerged then we would pass that on to the clergy/safeguarding officer but it was more a question relating to personal details. For example, how much detail concerning people’s medical conditions and also family dynamics should we be recording?
Do other pastoral care visitors ask the visitee’s permission to pass on information following a visit?
All views most welcome!

I keep what I record to the minimum necessary, using only initials of those I visit. Any important issues, such as safeguarding or health concerns, I discuss in private with my priest asking permission only if this is necessary.
